REFERRAL LETTERS

When referring patients it is important to:
  • Explain to the patient why you want to refer them and to whom
  • Examine the radiographs and clinical findings
  • Take a new medical history
The following information should be contained in a referral letter.
  • Patients name, address, and date of birth (Date of Birth)
  • Any relevant medical or social history
  • Reason for the referral
  • Any relevant treatment carried out and the dates
  • History of the complaint
  • Details on any vital notes such as radiographs
  • Name and contact detail of person to whom you are referring the patient

ORTHODONTIC.
  • Radiographs and study models
  • Previous orthodontic treatment
  • Caries rate
  • Oral hygiene
  • Patient’s attitude to treatment
  • Diagnosis
ORAL SURGERY REFERRALS
  • Relevant radiographs
  • Indication of urgency
  • Suggested diagnosis
  • History of complaint
PERIODONTAL REFERRALS
  • Oral Hygiene history
  • Relevant Radiographs
  • Information on Deep or persistent pockets
  • Mobility scores
  • CPITN Score
