- Examine the radiographs and clinical findings
- Take a new medical history
- Patients name, address, and date of birth (Date of Birth)
- Any relevant medical or social history
- Reason for the referral
- Any relevant treatment carried out and the dates
- History of the complaint
- Details on any vital notes such as radiographs
- Name and contact detail of person to whom you are referring the patient
